In the rehabilitation of elderly and senile patients, physical therapy (PT) plays a significant role; physical activity of such patients is necessary. Therapeutic gymnastics, recreational walking, dosed exercises on exercise machines, swimming, etc. are prescribed from exercise therapy. An individual approach and justification for prescribing any means (type) of exercise therapy are required, taking into account the patient’s main and concomitant diagnoses. The complex of exercise therapy is prescribed by a doctor who has a specialization and certificate in exercise therapy and massage.
Taking into account age-related changes and the aging process of the body, some features of prescribing physical therapy in old and senile age are highlighted:
1. The dosage should be lower than in middle-aged and young patients;
2. Exercises of low and medium intensity are used, with a fairly wide range of motion, and they are performed smoothly;
3. Attention is paid to the development and organization of correct breathing;
4. The principle of gradualism is strictly observed, pauses for rest are increased;
5. Exercises with sharp turns and rotations of the head, elements of straining and holding the breath, throwing back the head and tilting the body upside down are excluded;
6. Exclude or limit exercises for strength, speed, agility;
7. At the first signs of fatigue, physical activity should be stopped immediately;
8. The selection, volume and method of application of exercises should be such as to give patients a feeling of satisfaction and a desire to continue exercising.
2.2.5 Psychological correction
Psychological correction is a directed psychological impact on certain psychological structures in order to ensure the full development and functioning of the individual. Psychological correction as a targeted psychological impact is implemented not only in medicine, but also in other areas of human activity. It is also an important psychoprophylactic tool. Psychotherapy is a type of interpersonal interaction in which the patient is provided with professional assistance through psychological means in solving problems or difficulties that arise. psychological nature. Psychotherapy in gerontological practice represents a complex of psychotherapeutic measures aimed at restoring and activating physical, mental and social functions, skills and capabilities, as well as to solve specific problem situations that an elderly patient cannot cope with on his own. In geriatric clinics and special institutions for older people, group psychotherapeutic methods are used. The goals of group psychotherapy for older people are to involve them in social influence, increase self-esteem, strengthen independence, and focus on an ever-changing reality. Various group techniques are used: group discussion, music therapy, dance therapy, psycho-gymnastics and others. Family counseling is of great importance in gerontopsychological practice.
Psychocorrection and psychotherapy are important components of medical and psychological rehabilitation, aimed at the full or partial restoration of the patient’s personal and social status.
Group forms of psychotherapy can be considered the most adequate method of psychological rehabilitation in geriatrics. Group psychotherapy, more than any other method of psychosocial influence, helps restore the system of relationships between elderly patients and their microsocial environment, bringing value orientations into line with their lifestyle. There are two types of group methods of psychotherapy and sociotherapy in rehabilitation.
1. Therapeutic procedures aimed at the patient’s social behavior, his communication skills, the ability for self-realization, resolving psychological and overcoming social conflicts.
2. Optimal organization of the social structure of a group of patients, based on so-called environmental groups: functional groups, patients’ club. These sociotherapeutic groups are focused on the social activation of patients and their involvement in life in society, promote communication training and instill in patients the skills of adequate behavior. They provide a corrective social climate that allows one to relive interpersonal connections.
2.2.6. Art therapy
Art therapy is defined as therapy through art and creative expression. The term “art therapy” was first used by A. Hill in 1938 to describe his work with tuberculosis patients.
The etymology of the term “art therapy” has some ambiguity when used in Russia. There is an opinion that art therapy includes all types of creativity, including music therapy, drama therapy, dance therapy, etc. This point of view is based, perhaps, on the consonance of the term “art therapy” with the word “artistry”. In English-speaking countries, the word “art” means primarily visual arts: painting, graphics, photography, sculpture, design, or such forms of creativity in which the visual communication channel plays a leading role. In Russian literature you may come across the term “isotherapy” or “art therapy”. They are not identical to the English original and somewhat narrow its meaning. The closest definition to the English one would be: “art therapy is a method of psychotherapy that involves the client using various forms of visual activity to express his mental state.”
Art therapy can help solve the following problems.
1. Provide a socially acceptable outlet for aggression and other negative feelings. Working on drawings, paintings, sculptures is in a safe way"let off steam" and relieve tension.
2. Facilitate the treatment process. Unconscious internal conflicts and experiences are often easier to express through visual images than to express them in the process of verbal psychotherapy. Nonverbal communication more easily escapes the censorship of consciousness.
3. Obtain material for interpretation and diagnostic conclusions. Products of artistic creativity are relatively durable and the patient cannot deny the fact of their existence. The content and style of the artwork provides the therapist with a wealth of information about the patient, who can help interpret his own artwork.
4. Work through thoughts and feelings that the patient is used to suppressing. Sometimes nonverbal means are the only way to express and clarify strong feelings and beliefs.
5. Establish a relationship between therapist and patient. Participating in artistic activities together can help create relationships of empathy and mutual acceptance.

8. Develop artistic abilities and increase self-esteem. A by-product of art therapy is the feeling of satisfaction that comes from identifying hidden talents and developing them.
9. In addition to what was described above, art brings joy, and this is important in itself, regardless of whether this joy remains in the depths of the subconscious or is the result of realizing the opportunity to have fun, or even fool around.
Types and technologies of art therapy used in geriatrics.
Group art therapy has a number of advantages: it allows you to work with a wider range of clients and is more “economical”. Group art therapy allows you to develop valuable social skills; is associated with the provision of mutual support by group members and allows them to solve common problems; gives you the opportunity to observe the results of your actions and their impact on others; allows you to master new roles and demonstrate latent personality traits, as well as observe how modification of role behavior affects relationships with others; increases self-esteem and leads to strengthening of personal identity; develops decision-making skills.
Studio open group. The word “open” in the name of this group means that it does not have a permanent composition. At any stage, new members can join the group. This group is called a studio group because its working conditions are somewhat reminiscent of an art studio. The studio open group is one of the earliest forms of art therapy work. The work of open studio groups sometimes resembles occupational therapy or industrial therapy, especially in cases where it is associated with the production of arts and crafts and includes another “productive” element.
An analytical closed group is focused mainly on working with the unconscious mental processes of its participants. The word “closed” in its name means that throughout its operation its composition remains constant. The development of those forms of art therapy work that are associated with the concept of an analytical group was influenced by different directions of the psychodynamic approach.
The analytical group combines both visual activity of participants and verbal communication. The verbal component can vary over a wide range: from spontaneous statements commenting on the visual process to group discussion. Group analytical sessions are very dynamic and at different stages include either deep immersion of participants in independent work or their inclusion in a collective discussion.
The analytical closed group is designed primarily to work with patients with borderline mental disorders, or with persons who do not have any clinical disorders, but experience certain psychological or social problems. Analytical groups can be used in working with families, with various small communities of people united on any basis, in working with offenders, in some cases, with persons undergoing treatment for alcohol or drug addiction and other categories of clients.
A thematically oriented group, compared to studio and analytical groups, requires a greater degree of structure. This is achieved through the use of certain topics that address the common interests and problems of the participants. In most cases, topic-oriented groups are semi-open. Some members may leave the group at a certain stage. New members can also join at any time. This is possible because the work of a thematically oriented group does not have the same dynamics that are characteristic of an analytical one. As a rule, a topic-oriented group works for a short time, focusing on certain aspects of the experience of its participants and solving more or less specific problems. This group is also distinguished by the fact that it attaches importance to both the social factors influencing the work of its members and the action of internal healing factors associated with unconscious mental processes and creativity.
The specific objectives of art therapy in working with older people are overcoming social isolation, increasing the self-esteem of an elderly person, creating conditions for updating his life experience, recognizing his values, realizing his creative potential, and understanding his intrapersonal conflicts. Elderly people in most cases have somatic diseases, and therefore require a preliminary assessment of their physical condition. Art therapy work with them can be structured in different ways, but it is advisable to give preference to group forms.
Working in an art therapy group with older people is difficult, as they have difficulty verbalizing thoughts and feelings. Many cannot distinguish a thought from a feeling; they believe that they are the same thing. Some people feel afraid of speaking in public, always wait for grades, feel embarrassed, worrying that they cannot draw. Sometimes it can be difficult to break existing stereotypes and convince patients to do something new for them. However, many are unaware of their hidden creative abilities.
When working with older people, it is advisable to focus on positive memories and a review of life’s path.
At the same time, the emphasis is often placed on the active side of working with older people, and not at all on giving them the opportunity to be themselves and do what they want - to freely express themselves in drawings in each individual style and pace. It is very important that elderly patients follow their inner needs.
2.2.7. Spa treatment
Before making a decision to send an elderly person to a sanatorium-resort treatment, it is necessary to conduct a particularly thorough clinical examination using various functional diagnostic methods in order to exclude hidden diseases and correctly assess the patient’s functional capabilities.
The mechanism of the therapeutic effect of resort factors is quite complex. It is due to a whole complex in which the importance of balneological procedures, climate, intake of mineral waters, rational nutrition with the use of therapeutic diets is very important.
Changes in the situation and the distraction factor are very important. In this regard, the question can and should often be raised about the advisability of sending older people to resorts, in particular, balneological resorts with good climatic conditions, even in cases where the use of the main therapeutic factor of the resort is contraindicated (mud, brine, sulfur, narzan baths), but other, no less important healing factors can be successfully used.
